Does Healthy Blue Cover ABA Therapy?
Yes. Healthy Blue covers ABA therapy for autism under NC Medicaid's rules when it's medically necessary, with no copay, and the plan must approve it before therapy starts. Alpaca Health accepts Healthy Blue, checks your benefits for free, handles the approval paperwork with your BCBA, and doesn't have a waitlist.

How Healthy Blue covers ABA therapy
Healthy Blue pays for ABA as part of NC Medicaid's autism benefit.
- The benefit's name: NC Medicaid calls it research-based behavioral health treatment (RB-BHT), and ABA is one of the covered methods.
- Where the rules live: NC Medicaid's Clinical Coverage Policy 8F, updated August 1, 2026, sets the ABA rules, and Healthy Blue uses state-approved policies to decide whether ABA is medically necessary (needed to treat your child's autism).
- A Blue Cross NC Medicaid plan: Healthy Blue is one of NC Medicaid's Standard Plans, offered statewide by Blue Cross and Blue Shield of North Carolina.
- Same policy across NC Medicaid: Healthy Blue follows the same autism policy as the rest of NC Medicaid, so its members get the same ABA benefit.
- Children under 21: Under EPSDT (the federal rule that Medicaid covers the care children need), Healthy Blue can't turn down a child's request just because of a plan limit.
- Not sure which plan you have: We check your child's Medicaid plan during your free benefits review.
What your child needs to qualify
1. An autism diagnosis
A doctor, a licensed psychologist, or a licensed psychological associate must diagnose autism. They use a research-backed tool such as the ADOS-2 (a play-based autism test).
2. A provisional diagnosis for children under 3
A child under 3 can start ABA with a provisional (working) diagnosis when signs of autism are strong. A full diagnosis has to follow within six months for ABA to go on.
3. A skills and behavior assessment
Before the first approval, the BCBA tests your child's skills and daily living skills with standard tools. For challenging behaviors, the BCBA also looks at why they happen.
4. A treatment plan that includes you
Your BCBA, who must hold a North Carolina license, writes the treatment plan. It sets goals for your child, at least two goals for you, and a plan to lower hours over time.
5. A signed service order
Before therapy starts, a doctor or licensed psychologist signs a service order, a written order for ABA. It lasts one year, and each new order confirms your child still needs therapy.
How prior authorization works with Healthy Blue
This step means getting Healthy Blue's approval before an ABA service starts, even the first assessment. The plan's member handbook lists autism treatment, such as ABA, among the services it must approve first.
- What your BCBA sends: For therapy, the BCBA sends the assessment, the treatment plan, and the hours requested, and a signed service order must be in place.
- How fast Healthy Blue decides: A standard request gets a decision within 14 days, and an urgent one within 3 days.
- How long approval lasts: Approval covers up to 180 days when therapy runs 16 hours a week or less, and up to 90 days for more hours.
- Renewals: Each reauthorization (a renewed approval) needs an updated treatment plan, sent before the current approval ends.
- If the answer is no: You have 60 days from the date on the letter to ask for an appeal, and Healthy Blue has 30 days to answer.
- Step 1DiagnosisA psychologist or doctor confirms autism with a research-backed tool such as the ADOS-2.
- Step 2Assessment approvalHealthy Blue approves the BCBA's assessment hours (CPT 97151).
- Step 3Treatment plan and orderThe BCBA sets goals for your child and family, and a doctor or psychologist signs the order.
- Step 4Therapy approvalHealthy Blue approves weekly hours for up to 90 or 180 days.
- Step 5Progress reviewsAn updated plan goes back to Healthy Blue before each approval ends.
What Healthy Blue covers for ABA therapy
Your BCBA asks Healthy Blue for hours under the billing codes (CPT codes) below. Healthy Blue's provider manual sets approval rules for this treatment. Each code needs approval before the plan pays.
- Assessments (97151, 97152): The BCBA tests your child's skills and writes the plan, and a technician can help with some of the testing.
- One-to-one therapy (97153): A behavior technician works with your child in person, and new technicians have 120 days to earn an RBT or ABAT certification.
- Group sessions (97154): A technician leads group sessions in person when the treatment plan calls for them.
- BCBA sessions (97155): The BCBA works with your child or coaches the technician to adjust the plan, and up to half of these hours can happen by video, or more if the treatment plan explains why.
- Caregiver training (97156, 97157): Coaching for parents and caregivers, with at least six sessions every 180 days.
NC Medicaid's policy sets no weekly hour limit. Your BCBA offers a range of weekly ABA hours based on your child's needs. When your child is at school, that time doesn't count toward those hours.
What families pay with Healthy Blue
Covered ABA has no copay. Under NC Medicaid's copay rules, behavioral health care and anyone under 21 have no copays. There's no monthly premium either.
- Nothing owed for covered care: Healthy Blue's handbook says you won't pay for care that it or Medicaid covered, even if the plan later denies the provider's claim.
- When you could owe: You may have to pay for a service Healthy Blue didn't approve, or one you agreed ahead of time to pay for yourself.
- Other insurance pays first: If your child also has private insurance, providers bill that plan before Medicaid.

Book a consultationView profileDoes Healthy Blue require prior authorization for ABA therapy?
Yes, Healthy Blue must approve ABA before each service starts, from the first ABA assessment to weekly therapy. Its reviewers check for medical necessity (whether your child needs ABA to treat autism). If you still disagree after an appeal, a judge who isn't part of Healthy Blue can review it at a State Fair Hearing. Alpaca Health handles the approval paperwork with your BCBA.
Is Healthy Blue the same as Blue Cross NC?
Healthy Blue is an NC Medicaid plan offered by Blue Cross and Blue Shield of North Carolina (Blue Cross NC). As a Medicaid plan, it follows NC Medicaid's ABA policy, and covered ABA has no copay. If your child has a Blue Cross NC plan that isn't Medicaid, the ABA rules for Blue Cross Blue Shield plans apply instead.
Does Healthy Blue Care Together cover ABA therapy?
Yes, Healthy Blue Care Together lists autism treatment in its provider manual, and the plan must approve it first. It's NC Medicaid's statewide Children and Families Specialty Plan, run by Blue Cross NC since December 1, 2025. It's for children and young adults who are or have been in the child welfare system. Providers join it through its own contracting process, so we check that your child's BCBA is in network for it.
Does Healthy Blue cover autism testing?
Yes, Healthy Blue pays for testing by a psychologist. The plan has to approve it only after a set number of visits. Checkups for children include screenings that look at language and social skills, but a screening alone can't start ABA. If your child doesn't have a diagnosis yet, Alpaca Health offers an autism evaluation and checks your coverage first.
Does Healthy Blue cover ABA therapy for adults?
Yes, and the benefit doesn't stop at 21. A federal approval let NC Medicaid cover RB-BHT for members over 21 starting July 1, 2021. Research has to back the treatment for the person's age. EPSDT's extra protections end at 21 in every state's Medicaid program, so adults follow the standard policy alone.
