1. An autism diagnosis
A licensed clinician, such as a psychologist or pediatrician, must diagnose autism. Michigan's Blue plan wants the evaluation done by a team of specialists. If your child doesn't have a diagnosis yet, Alpaca Health offers an autism evaluation and checks your coverage first.
2. A recent standardized assessment
A BCBA tests your child's skills with a standard tool, such as the VB-MAPP or Vineland. The Texas Blue plan wants results from the past 30 days. Blue Cross NC accepts results up to 60 days old.
3. A treatment plan with measurable goals
The BCBA writes a treatment plan with clear goals and starting data for each one. All four Blue plans also ask how parents or caregivers will take part.
4. A qualified provider
A BCBA, a licensed behavior analyst, or another licensed clinician trained in ABA must provide or supervise therapy. The Michigan and North Carolina Blue plans cover ABA at any age, while Nebraska law covers children under 21.
How prior authorization works with Blue Cross Blue Shield
This step means getting your Blue plan's approval before therapy starts. Your BCBA usually sends that request along with your child's assessment results and treatment plan.
- Texas: Blue Cross and Blue Shield of Texas requires approval for ABA, and its ABA request form should arrive at least two weeks before therapy starts.
- Michigan: Every Blue Care Network HMO plan requires approval, and some PPO plans do too, with Blue Cross Behavioral Health making the decision.
- Nebraska: Blue Cross and Blue Shield of Nebraska requires approval under its autism policy, and from November 15, 2026, it also wants a doctor's referral from the past three years.
- North Carolina: Blue Cross NC has required approval for ABA on its commercial plans since 2022, and it checks each request against its ABA policy.
- Renewals every six months: Each reauthorization (a renewed approval) needs new progress data, about every six months on all four plans.

What Blue Cross Blue Shield covers for ABA therapy
The Texas and North Carolina Blue plans both list the ABA billing codes below. You'll see them on your explanation of benefits.
- Assessments (97151, 97152): Testing and treatment planning, done again about every six months.
- One-to-one therapy (97153): A behavior technician works with your child under the BCBA's direction.
- BCBA supervision (97155): The BCBA adjusts the program and guides the technician.
- Caregiver training (97156, 97157): Coaching for parents and caregivers, alone or with other families.
- Group sessions (97154, 97158): Small-group therapy when the treatment plan sets group goals.
Nebraska's Blue plan caps ABA at 25 hours a week starting November 15, 2026, as state law allows. The Michigan and North Carolina Blue plans' ABA rules set no weekly cap.

Does Blue Cross Blue Shield cover in-home ABA therapy?
