Does HMSA Cover ABA Therapy?
Yes. HMSA, Hawaii's Blue Cross Blue Shield plan, covers ABA therapy for autism when it's medically necessary, and ABA services need HMSA's approval before they start. Alpaca Health accepts HMSA, checks your benefits for free, and handles the approval paperwork with your BCBA.

How HMSA covers ABA therapy
HMSA pays for ABA once a licensed expert diagnoses autism and a BCBA's testing shows your child needs it. In HMSA's terms, the therapy must be medically necessary.
- Where the rules live: HMSA's criteria are in its ABA medical policy, and your plan contract wins if the two differ.
- What plan guides say: HMSA guides, such as its Preferred Provider Plan guide, cover ABA for autism from a behavior analyst HMSA recognizes, in the state where you live.
- Fully insured plans: In these plans HMSA pays the claims, and Hawaii's autism law adds rules, shown in the table further down.
- Self-funded employer plans: Your employer pays the claims, and these plans follow their own plan documents instead of state autism laws, as federal guidance explains.
- Not sure which you have: Your HR team can tell you, or we check during your free benefits review.
- HMSA QUEST: This is HMSA's Medicaid plan, so Hawaii's Med-QUEST rules apply, and ABA is covered for members under 21.
What your child needs to qualify
1. A screening by your child's doctor
Your child's regular doctor first checks for delays in growth, talking, and behavior, then makes the diagnosis or refers you to a specialist. Neither visit needs HMSA's approval.
2. An autism diagnosis
A pediatrician, psychologist, or another licensed expert confirms autism with standard tests. If your child doesn't have a diagnosis yet, Alpaca Health offers an autism evaluation and checks your coverage first.
3. An assessment and a treatment plan
A BCBA tests your child's skills and writes a treatment plan with goals for social skills, language, behavior, and daily living. Any behavior of concern also gets a functional behavior assessment to find what sets it off.
4. A plan for parent training
HMSA asks for a written plan to coach parents or caregivers, with goals you can measure. Renewals check that your family uses what you learned at home.
5. A licensed provider in Hawaii
Hawaii's autism law only covers care from a provider licensed by the state. HMSA also covers ABA only in the state where you live, so care on the mainland isn't covered unless you live there.
How prior authorization works with HMSA
This step means getting HMSA's approval before ABA services start. HMSA calls this precertification. The assessment and every stretch of therapy need it, but screening and the diagnostic visit don't.
- Two separate requests: Your BCBA asks HMSA to approve the assessment first, then sends a separate request for treatment.
- What the treatment request includes: The diagnosis, assessment results, treatment plan, weekly hours, a parent training plan, and a sample weekly schedule go to HMSA.
- When HMSA decides: HMSA answers a routine request within 15 days and can extend that once by another 15 days.
- Most approvals last up to 26 weeks: Each reauthorization (a renewed approval) goes in at least two weeks before the current one ends, with progress data from the past 30 days.
- If progress stalls: For each goal with no progress, the BCBA explains why and changes the strategy or the goal.
- Step 1DiagnosisYour child's doctor screens, then a licensed expert confirms autism.
- Step 2Assessment approvalHMSA approves the BCBA's assessment before testing starts.
- Step 3Treatment planThe BCBA sets measurable goals, including goals for parent training.
- Step 4Therapy approvalHMSA reviews the plan and approves weekly hours for up to 26 weeks.
- Step 5Progress reviewsBefore each renewal, progress data goes back to HMSA.
What HMSA covers for ABA therapy
HMSA's ABA policy lists the billing codes below, and each one shows up on your explanation of benefits. Therapy can take place at home, in a clinic, or in the community. Some BCBA services can also happen by live video, but not by phone alone.
- Assessments (97151, 97152): Testing and treatment planning, approved on their own before therapy begins.
- One-to-one therapy (97153): A registered behavior technician (RBT) works with your child under a BCBA at an autism clinic in HMSA's network, and a BCBA in solo practice runs the sessions directly.
- BCBA program changes (97155): The BCBA works with your child, adjusts the program, and can guide the technician during the session.
- Caregiver training (97156, 97157): Coaching for parents and caregivers, alone or with other families.
- Group sessions (97154, 97158): Small-group therapy when the BCBA shows your child is ready to learn in a group.
- Destructive behavior (0362T, 0373T): Assessment and treatment with two or more technicians and a BCBA on site.
HMSA's policy sets no weekly hour limit. Instead, your child's assessment data has to show why the requested hours are needed.
State limits on fully insured HMSA plans
| State | What state law allows |
|---|---|
| Hawaii | Plans must cover autism diagnosis and treatment for children under 14, and may cap ABA at $25,000 a year (HRS 432:1-614). |
Self-funded employer plans follow their own plan documents. We checked these laws in October 2026.
What families pay with HMSA
Your bill depends on your plan's deductible, copay, and out-of-pocket maximum. The deductible is what you pay each year before your plan starts to share costs. After that, HMSA calls your share of each bill a copayment. It can be a flat amount or a percent of the bill.
- Same cost share as other care: Hawaii law says plans can't charge more for autism care than for most other medical care.
- Find your ABA line: HMSA's Guides to Benefits list ABA on its own line in the payment chart, and HMO and PPO plans set different shares.
- Network matters: In HMSA's PPO plans, ABA from a nonparticipating (out-of-network) provider costs you a bigger share than ABA from a participating one.
- Plan a full year: Once you know your deductible and any yearly ABA limit, you can estimate a year of ABA therapy costs.

Book a consultationView profileDoes HMSA require prior authorization for ABA therapy?
Yes. Every ABA billing code is on HMSA's list of services that need approval before they start. That includes the first ABA assessment and each stretch of therapy. Alpaca Health prepares the request with your BCBA, so you don't have to.
How many hours of ABA therapy does HMSA cover?
HMSA doesn't set a fixed weekly limit, so each request lists the hours per week your child needs. A standard approval covers up to 26 weeks. On employer plans and plans you buy yourself, HMSA can approve up to 52 weeks for 10 or fewer hours a week of one-to-one therapy. The BCBA can also ask for a separate approval for school breaks.
Does HMSA cover ABA therapy for teens?
Hawaii's autism law only requires coverage for children under 14. HMSA's ABA policy doesn't add an age cutoff, so your plan documents decide for teens. We check those documents during your free benefits review. On HMSA QUEST, ABA stays covered until age 21 under EPSDT (Medicaid's benefit for children).
Can my child start ABA with HMSA before an autism diagnosis?
It can, if the diagnosing expert suspects autism but needs more time to confirm it. In that case, HMSA's ABA policy allows a trial of ABA for up to 26 weeks at a time. Children with global developmental delay or a language disorder can qualify, and the BCBA updates the plan at least every 12 weeks. HMSA's plans for federal employees don't cover this trial.
Does HMSA QUEST cover ABA therapy?
Yes, HMSA QUEST covers ABA for members under 21 who need it. It's part of Medicaid, so the state's Med-QUEST guidelines and Medicaid rules both apply. HMSA's QUEST handbook says most benefits come at no cost, though the state sets a monthly cost share for some members. Children under 3 in Hawaii's Early Intervention Program get services there first, and the program works with HMSA when your child moves to ABA.
