How Carolina Complete Health covers ABA therapy
NC Medicaid pays for ABA as part of research-based behavioral health treatment (RB-BHT), and this plan uses the state's rules.
- Where the rules live: NC Medicaid's RB-BHT policy, updated August 1, 2026, sets who qualifies and what's covered, and Carolina Complete Health uses it to review ABA requests.
- A Standard Plan: Carolina Complete Health is one of NC Medicaid's Standard Plans, the kind of health plan most members get, and RB-BHT is on its list of covered services.
- Statewide since April 2026: WellCare of North Carolina merged into the plan on April 1, 2026, and Carolina Complete Health now serves all six Medicaid regions.
- Children under 21: Under EPSDT (the federal rule that Medicaid covers the care children need), the plan can't turn down a child's request just because of a plan limit.
- Not sure which plan you have: We check your child's Medicaid plan during your free benefits review.
What your child needs to qualify
1. An autism diagnosis
A licensed psychologist, a licensed psychological associate, or a doctor must diagnose autism. They must use a research-backed tool such as the ADOS-2 (a play-based autism assessment).
2. A provisional diagnosis for children under 3
A child under 3 can start ABA with a provisional (working) diagnosis based on strong signs of autism. A full diagnosis must follow within six months to keep ABA going.
3. A signed service order
A doctor or licensed psychologist signs an order for ABA before therapy starts. The order lasts one year, and your child's need for ABA is checked again each year.
4. A skills and behavior assessment
The BCBA tests your child's skills and daily living skills with tools such as the VB-MAPP and Vineland-3. For challenging behaviors, the BCBA also studies why they happen.
5. A treatment plan that includes you
Your BCBA writes the treatment plan and must hold a North Carolina license to do it. The plan sets goals for your child, at least two goals for you, and steps for lowering hours over time.
How prior authorization works with Carolina Complete Health
This step means getting the plan's approval before ABA services start. The plan's review guidelines require it for each covered ABA code, at the start and at each renewal.
- What your BCBA sends: For therapy, the signed service order, assessment results, and treatment plan go to the plan with the hours requested for each code.
- When the plan decides: A standard request gets a decision within 14 days, and an urgent one gets an answer within 3 days.
- How long approval lasts: Plans of 16 hours a week or less can be approved for up to 180 days, and since August 1, 2026, plans over 16 hours get up to 90 days.
- Renewals: Each reauthorization (a renewed approval) needs an updated plan with progress data, sent before the current approval ends.
- If the answer is no: You have 60 days from the date on the letter to ask for an appeal, and the plan has 30 days to answer.
- Step 1DiagnosisA psychologist or doctor confirms autism with a research-backed tool.
- Step 2Assessment approvalCarolina Complete Health approves the BCBA's assessment hours (CPT 97151).
- Step 3Order and treatment planA doctor or psychologist signs the order, and the BCBA sets goals for your child and family.
- Step 4Therapy approvalCarolina Complete Health approves weekly hours for up to 90 or 180 days.
- Step 5Progress reviewsUpdated plans and progress data go back to Carolina Complete Health before each renewal.
What Carolina Complete Health covers for ABA therapy
Your BCBA asks for hours under the billing codes (CPT codes) below. The plan must approve each code before it pays.
- Assessments (97151, 97152): The BCBA's testing and treatment planning, with a technician's help for some tests.
- One-to-one therapy (97153): A behavior technician works with your child and must earn an RBT or ABAT certification within 120 days of being hired.
- Group sessions (97154): Small-group therapy run by a technician when the treatment plan calls for it.
- BCBA sessions (97155): The BCBA works with your child or guides the technician during a session to adjust the plan, and up to half of this time can happen by video.
- Caregiver training (97156, 97157): Coaching for parents and caregivers, with at least six sessions planned for every 180 days.
The BCBA suggests a range of weekly ABA hours based on your child's needs. NC Medicaid doesn't allow default hours, and providers can't require a minimum unless your child's goals call for it. Time at school doesn't count toward ABA hours.
What families pay with Carolina Complete Health
Covered ABA has no copay. NC Medicaid's copay rules exempt all behavioral health services and all members under 21. Medicaid also doesn't charge monthly premiums.
- Nothing to pay for covered care: Under the plan's member handbook, you won't have to pay for care it covered, even if it later denies the provider's claim.
- Other insurance pays first: If your child also has private insurance, providers must bill that plan before Medicaid.
- Keep coverage active: Return your Medicaid renewal forms on time so your child's coverage doesn't have a gap.

Does Carolina Complete Health require prior authorization for ABA therapy?
Yes, the plan must approve each covered ABA code before it's used, from the first ABA assessment to parent training. Reviewers check for medical necessity (whether your child needs ABA to treat autism). Alpaca Health handles that paperwork with your BCBA.
Did WellCare of North Carolina become Carolina Complete Health?
Yes, the two plans merged on April 1, 2026, and WellCare members moved over automatically. Benefits didn't change, so NC Medicaid ABA coverage stayed the same. ABA approvals that were open on that date carried over, and BCBAs in WellCare's network joined the new plan's network.
Does Carolina Complete Health cover autism testing?
Yes, the plan covers psychological testing. In-network providers need its approval only after the first 16 units of testing. Well-child checkups include developmental screening, but a screening alone can't start ABA. If your child doesn't have a diagnosis yet, Alpaca Health offers an autism evaluation and checks your coverage first.
Does Carolina Complete Health cover ABA therapy for adults?
Yes, NC Medicaid has covered RB-BHT for members over 21 since July 1, 2021, when research supports the treatment for their age. The current policy sets no upper age limit. EPSDT, the federal rule that makes every state's Medicaid program cover the care children need, stops at 21. After that, adult requests follow the standard policy alone.
Can my child get ABA at home or at school with Carolina Complete Health?
Yes, NC Medicaid allows ABA in a clinic, at school, in the community, or through in-home ABA sessions. The BCBA picks settings with your family. ABA can't replace a teacher or aide, and it can't repeat services in your child's IEP (special education plan). You can ask for in-person sessions any time, since video sessions are never required.
