Does AmeriHealth Caritas North Carolina Cover ABA Therapy?
Yes. AmeriHealth Caritas North Carolina covers ABA therapy for autism when it's medically necessary, with no copay, and each ABA service needs the plan's approval before it starts. Alpaca Health accepts AmeriHealth Caritas North Carolina, checks your benefits for free, and handles the approval paperwork with your BCBA.

How AmeriHealth Caritas North Carolina covers ABA therapy
This plan pays for ABA as part of NC Medicaid's autism benefit.
- What Medicaid calls it: NC Medicaid's name for this benefit is research-based behavioral health treatment, or RB-BHT.
- Where the rules live: NC Medicaid's Clinical Coverage Policy 8F, updated August 1, 2026, sets the ABA rules, and the plan uses state-approved policies to decide what's medically necessary.
- A statewide Medicaid plan: AmeriHealth Caritas North Carolina is one of NC Medicaid's Standard Plans, which cover physical health, pharmacy and basic behavioral health care.
- More than ABA: The same benefit covers other autism therapies with strong research behind them, such as the Early Start Denver Model.
- Extra protection under 21: Because of EPSDT (the federal rule that Medicaid covers the treatment children need), the plan can't deny a child's care just because of a policy limit.
- Where therapy happens: Sessions can take place at home, in a clinic, at school or in the community, and your family helps choose the setting.
What your child needs to qualify
1. NC Medicaid through this plan
Your child needs active NC Medicaid with this plan as their health plan. There's no age cutoff for this benefit, since NC Medicaid covers adults as well as children.
2. An autism diagnosis
A doctor, licensed psychologist, or licensed psychological associate must diagnose autism with a standard test such as the ADOS-2 (a play-based autism test). A screening checklist or a school's decision can't start therapy by itself.
3. A provisional diagnosis under age 3
A licensed clinician can give an early, provisional diagnosis when signs of autism are strong but testing isn't done. Therapy can start with it, and a full diagnosis has to follow within six months.
4. An assessment and treatment plan
A BCBA with a North Carolina license, a licensed psychologist, or a doctor tests your child's skills and writes the treatment plan. The plan must include at least two goals for you as a caregiver.
5. A signed service order
Before therapy starts, a doctor or licensed psychologist signs a service order (a written order for therapy) based on the assessment and plan. The order lasts a year, and each new one confirms medical necessity, which means your child still needs therapy.
How prior authorization works with AmeriHealth Caritas North Carolina
This step means getting the plan's approval before an ABA service starts. RB-BHT is on the plan's prior authorization list, so even the first assessment needs approval.
- What your BCBA sends: For therapy, the plan gets the assessment, the treatment plan, the signed service order and the hours requested.
- How fast the plan decides: A standard request gets a decision within 14 days, and an urgent one within three days.
- How long approval lasts: Approval covers up to 180 days when therapy runs 16 hours a week or less, and up to 90 days for more hours.
- Renewals: Each reauthorization (a renewed approval) needs an updated treatment plan sent in before the current approval ends.
- Switching plans: If your child joins from another Medicaid plan, approvals already in place are honored for the first 90 days.
- Step 1DiagnosisA psychologist or doctor confirms autism with a standard test such as the ADOS-2.
- Step 2Assessment approvalAmeriHealth Caritas North Carolina approves the BCBA's assessment hours (CPT 97151).
- Step 3Treatment planThe BCBA sets measurable goals for your child and at least two for you.
- Step 4Therapy approvalA doctor or psychologist signs the service order, and AmeriHealth Caritas North Carolina approves weekly hours.
- Step 5Progress reviewsAn updated plan goes back to AmeriHealth Caritas North Carolina every 90 or 180 days.
What AmeriHealth Caritas North Carolina covers for ABA therapy
Each request your BCBA sends lists the billing codes (CPT codes) and hours for the services below.
- Assessments (97151, 97152): The BCBA tests your child's skills and writes the plan, and a technician can help with some of the testing.
- One-to-one therapy (97153): A certified behavior technician works with your child and follows the plan.
- Group sessions (97154): A technician leads small-group therapy when the plan calls for it.
- BCBA sessions (97155): The BCBA works with your child or guides the technician to adjust the plan.
- Caregiver training (97156, 97157): Coaching for parents happens one family at a time or in a group, with at least six sessions every 180 days.
NC Medicaid's policy sets no weekly hour limit, so the hours in your child's plan come from the assessment. Since an August 2026 update, technician sessions must happen in person. Video can cover up to half of the BCBA's 97155 hours.
What families pay with AmeriHealth Caritas North Carolina
Covered ABA has no copay. Under NC Medicaid rules, behavioral health care has no copays at any age. Members under 21 have no copays for any service. There's no monthly premium either.
- Copays on your plan: AmeriHealth Caritas North Carolina lists RB-BHT among its behavioral health services, which carry no copays.
- Bills for covered care: The plan's member handbook says a provider who accepts Medicaid generally can't bill you.
- When you could owe: You may have to pay for services the plan didn't approve, or if you agree ahead of time to pay on your own.
- Other insurance pays first: If your child also has private insurance, the provider bills it before Medicaid.

Does AmeriHealth Caritas North Carolina require prior authorization for ABA therapy?
Yes, the plan must approve every ABA service in advance, from the first ABA assessment to weekly therapy. If the plan says no, you can file an appeal. After that, you can ask for a State Fair Hearing, a review by a judge who doesn't work for the plan. Alpaca Health handles the approval paperwork with your BCBA.
How many hours of ABA therapy does AmeriHealth Caritas North Carolina cover?
There's no fixed weekly limit. Your BCBA suggests a range of hours a week based on your child's needs. Providers can't make you accept a minimum number of hours unless your child needs them to progress. Time at school or in IEP services doesn't count toward therapy hours.
Does AmeriHealth Caritas North Carolina cover autism testing?
Yes, the plan pays for testing by a psychologist. You don't need a referral for your child's first behavioral health assessment in a 12-month period. A licensed school psychologist's testing can count too, if it doesn't need to be redone. If your child doesn't have a diagnosis yet, Alpaca Health offers an autism evaluation and checks your coverage first.
Does AmeriHealth Caritas North Carolina cover ABA therapy for teens and adults?
Yes, and the benefit doesn't end at 21. NC Medicaid has covered RB-BHT for members over 21 since July 1, 2021, under a federal approval. The therapy needs research support for the person's age. Teens and adults ask the plan for approval the same way children do. The policy allows a focused plan that works on a few skills, such as social skills for teens.
Does my child need a Tailored Plan to get ABA therapy?
No, this plan covers RB-BHT, so your child can get ABA without switching. Tailored Plans are for members with more complex needs, such as intellectual or developmental disabilities or serious mental illness. They're the only plans that offer the NC Innovations Waiver. If your child qualifies, you can ask to move to your county's Tailored Plan, such as Alliance Health or Vaya Health.
