What do you need from us to refer?

Your contact information, the patient's name and date of birth, and a parent or guardian phone number. The form below covers all of it. If the family's insurance requires a signed referral, you can send it by fax.
What insurance do you accept?

Medicaid and most major commercial plans. We verify each family's benefits during intake, so your office does not need to check coverage before referring.
What ages do you serve?
